Request for Completed Financing Questionnaire

If you are a member of or an attorney who represents a member of a client of Spinnaker Management, L.L.C. and your financing application requires a completed questionnaire, please mail or deliver the following to:

All questions should be addressed to our office manager at (845) 895-8122 or OfficeManager@spinnakerusa.com.

1. A written request for a completed Association questionnaire.
   1. Please include the Association/property name, unit number and/or street address on the request.
2. A check made payable to Spinnaker Management, L.L.C. for the preparation of the questionnaire. The fee is $75.00.
3. The person, address and/or fax where the completed questionnaire should be mailed or faxed. (The completed questionnaire will not be returned until payment is received.)
